Hospice is not a place—it’s a service. Hospice brings physical, emotional, spiritual and other support care to wherever a patient calls home. Hospice care is patient-centered with the activities of the hospice team driven by the needs of the patient and their loved ones.

Hospice is not the hastening of death, rather it is about celebrating the time the patient has left, and making them as comfortable as possible.

The majority of Hospice Care takes place in the place the patient calls home, so they can be surrounded by family and loved ones. Hospice Care is about quality end-of-life care that helps the patient live every moment of every day they have left.

Patients are assisted with their Activities of Daily Living (ADLs), which include medication management, eating, bathing, dressing, toileting, walking, transferring and continence.

In-Home Personal Care

After the patient is transferred from the hospital to their home, at-home hospice staff manage medical orders and medications, physical therapy (PT) and occupational therapy (OT), as well as assisting with the patient’s Activities of Daily Living (ADLs).

  2. Medical Director: Oversees and advises the plan for care
  3. RN/LPNs: Assesses and manages pain and other symptoms to provide comfort and quality for end-of-life care
  4. Social Worker: Assesses and addresses the psychosocial needs of the patient and family, and coordinates community resources
  5. Spiritual Counselor: Works together with the care team and directly with the patient and family to assess and support spiritual needs
  6. Bereavement Counselor & Support: Grief is a very personal experience. The offered counseling is about recognizing and meeting people where they are in the grief process and helping them to develop coping strategies.
